Student Insurance 2009-2010: United Healthcare
Loyola's student health insurance provider for the 2009-2010 school year remains United HealthCare Insurance Company. Highlights and benefits:

These rates apply to Loyola students, their spouses and children. The student's annual or spring charges will be billed to his/her Loyola student account if he/she meets the eligibility requirements for automatic enrollment. Eligibility Requirements
Loyola University Chicago's Board of Trustees requires that all undergraduate students enrolled for 12 or more credit hours and all graduate and professional students enrolled for 8 or more credit hours (or registered for thesis or dissertation supervision) at the start of the fall or spring semester have individual health care insurance.
All students meeting the eligibility requirements will be enrolled in and charged for Student Health Insurance by the University to their student account. If you already have your own insurance or are covered under a parent's plan, you may waive out of Loyola's Student Health Insurance by providing proof of other coverage and a credit for the insurance charge will post to your student account.

Coverage Options
How To Waive Out of Loyola's Student Health Insurance:
If you have your own insurance already or are covered under your parent's plan, you may waive out of Loyola's Student Health Insurance by providing proof of the other coverage. Go to LOCUS and click on "Student Health Insurance" in the Campus Finances section. Submit the electronic waiver by October 1 for the Fall 2009 term and the insurance charge will be waived from your account.
How to Fast-Track Into Loyola's Student Health Insurance:
If you want Loyola's Student Health Insurance and already have the charge on your student account, you may Fast-Track into the coverage through LOCUS in the Campus Finances section. Fast-Tracking into the insurance verifies to United Healthcare that you are accepting the coverage so they process your enrollment and send you your insurance card before the October 1 Fall waiver deadline.
How to Purchase Supplemental Insurance Coverage:
Insurance coverage for yourself, a spouse, or a child can be purchased directly from United Healthcare.
In the event that a student does not meet the eligibility requirements to be automatically enrolled in Loyola's Student Health Insurance by the University, but would still like to purchase coverage, they can do so right through United HealthCare's Website at www.uhcsr.com/luc or by calling them at 1.800.767.0700.
To add a spouse or child onto the insurance plan, you can purchase supplemental coverage right through United HealthCare's Website at www.uhcsr.com/luc or by calling them at 1.800.767.0700.
